These templates are used where the law of the data subject's country requires consent as the basis for processing (for example Kazakhstan and Uzbekistan) or where a customer chooses to collect consent. They complement, and do not replace, the Privacy Policy (unitify.com/legal/privacy-policy) and the Data Processing Agreement (unitify.com/legal/dpa). Three templates are provided:

• Template A — Resident notice and consent: given by a resident or unit owner to the management company, homeowners' association or developer that uses Unitify (the "Manager"). The Manager is the controller. Unitify Limited is the processor or, under Orders contracted with its authorized distributor Unitify LLC (Uzbekistan), Unitify LLC is the processor and Unitify Limited is its sub-processor, as set out in the Data Processing Agreement.
• Template B — Account holder consent: given by a representative of a customer, partner or distributor when registering an account or signing a contract. The controller is Unitify Limited or, in the CIS, the authorized distributor that contracts with the organisation.
• Template C — Marketing consent: a separate, optional choice. Registration or signing a contract never implies marketing consent.

Terms in square brackets are completed by the Manager or at registration. Where a template conflicts with mandatory law of the data subject's country, that law applies.

Template A — Resident notice and consent

I, [full name], a resident or owner of [unit, building, address], confirm that I have read this notice and consent to the processing of my personal data by [Manager's legal name, registration number, address] (the "Manager") as described below. This consent covers my own data and the data of the minor [name], for whom I act as legal representative; it does not cover other adults, who confirm this notice themselves.

2. Processors

The Manager uses the Unitify platform. Under Orders contracted with Unitify Limited (Hong Kong), Unitify Limited is the processor; under Orders contracted with its authorized distributor Unitify LLC (Uzbekistan), Unitify LLC is the processor and Unitify Limited is its sub-processor. They process my data on the Manager's instructions under a Data Processing Agreement (unitify.com/legal/dpa). The data is hosted in [country of the service region]. The list of sub-processors (hosting, notifications, AI providers) is in Annex III of that Agreement. Payment providers chosen by the Manager process my payment data as independent controllers under their own terms.

3. Third parties and cross-border transfer

Transfer to third parties: only to the recipients named in Section 2 for the purposes in Section 1, and to public authorities where the law requires it. Cross-border transfer: [yes / no; countries or categories of recipients, for example the hosting country of the service region and the providers in Annex III]. Disclosure in publicly accessible sources: none. Where my data is transferred outside [country] (for example for push notifications or AI features), the Manager ensures the basis required by the law of [country]. Where the law requires my consent for such a transfer, I give it by signing this notice, and I may withdraw it (Section 6).

4. Retention

My data is kept for as long as I am a resident or owner of the unit and thereafter for the period the Manager must keep records under the law (billing and accounting records: [period]); request history and messages: [period] after closure; access and video recordings: [period]. Data no longer needed is deleted or anonymised. This consent is valid for as long as I am a resident or owner of the unit and for the retention periods above, unless I withdraw it earlier.

5. My rights

I may ask the Manager for access to my data, its correction, deletion where the law allows, restriction of processing, a copy of my data, and I may object to processing based on legitimate interest. I may lodge a complaint with [the competent authority of the country]. Requests: [Manager's contact]. If I write to Unitify, it forwards my request to the Manager.

6. Withdrawal of consent

I may withdraw this consent at any time by a written or electronic request to the Manager. Withdrawal does not affect processing carried out before it. After withdrawal the Manager stops the processing that was based on consent; it may continue to process data that it must keep or process under the law or to perform its obligations to the owners (for example billing records), and it informs me which data this concerns.

7. Confirmation

Signature / electronic confirmation in the resident app: [date]. By confirming I also acknowledge the Manager's privacy notice.

Template B — Account holder consent

I, [full name], [position] at [organisation] (the "Organisation"), confirm that I have read the Privacy Policy at unitify.com/legal/privacy-policy and consent to the processing of my personal data by [Unitify Limited, Hong Kong, Reg. No. 76842989 / Unitify LLC, Uzbekistan, TIN 311849914] (the "Controller") as described below.

1. Purposes and data

• Account and access — name, work e-mail, phone, position, organisation, role, login and activity logs — to create and maintain my account and the Organisation's account on the Platform.
• Contract and support — the same data, correspondence, invoicing details of the Organisation — to perform the agreement with the Organisation, provide support and invoice it.
• Security — technical and login data — to protect the Platform and the Organisation's account.

Marketing is not covered by this consent; it is a separate choice under Template C.

2. Recipients and transfers

Transfer to third parties: only to the service providers and recipients listed in the Privacy Policy (hosting, e-mail, support tools) and, in the CIS, to Unitify Limited for third-line support. Cross-border transfer: yes, to Hong Kong and to the countries where the Controller's staff and providers are located, as described in the Privacy Policy; where the law of [country] requires my consent for a cross-border transfer, I give it here and may withdraw it. Disclosure in publicly accessible sources: none.

3. Retention

For the life of my account and up to 3 years afterwards for the defence of claims; invoicing records for the period required by accounting law. This consent is valid for the same period unless I withdraw it earlier.

4. Rights and withdrawal

I may request access, correction, deletion where the law allows, restriction, a copy of my data, and withdraw this consent at any time at [email protected]. Withdrawal does not affect earlier processing. After withdrawal the Controller stops consent-based processing and may keep only the data required by law or for the defence of claims; without an account record the Organisation's administrator may need to appoint another contact person.

5. Confirmation

By clicking "Register" / "Sign Up" or by signing the agreement on behalf of the Organisation, I confirm this consent. Signature of the agreement covers only the person who signs it; a user added by the Organisation's administrator confirms this consent personally, in the form the Organisation uses (signature or electronic confirmation). I confirm that the e-mail address and phone number provided belong to me and that I am authorised to act for the Organisation.

Template C — Marketing consent (optional)

☐ I agree that [Controller] may use my name and e-mail address [and phone number or messenger identifier] to send me information about Unitify's software products (community and property management platform, resident app, billing), features, events and offers by e-mail [and messenger]. These data are not used for direct marketing before this consent and are not provided to third parties for their marketing. I may unsubscribe at any time using the link in every message or by writing to [email protected]. This choice is optional and does not affect my account or the Organisation's agreement. Marketing data is kept until I unsubscribe or for 3 years after my last interaction, whichever is earlier; after that only a record of my choice is kept.
